When you combine the three active ingredients in ProFlex, you get superior performance with the security of our ProFlex 90-day Mosquito Protection Program.
Apply one application of ProFlex at the below recommended application rate one time or about every 90 days per account. It is recommended to use one ounce of ProFlex in 1 gallon of water to treat 1000 ft2. Finished liquid amounts will vary based on the size of the account.
If any call-backs occur, CSI will provide the amount of ProFlex required to make the re-application to the treated area free of charge, per the guidelines set forth in the CSI ProFlex 90 Day Mosquito Protection Program.
Proflex 90-day Mosquito Protection Program dates are from April 1st through November 30, 2022.
Pest management professional (PMP) will provide a service ticket with the following information should a re-treatment be necessary within the 90-day interval:
• Date, time, location of the service and proof of purchase will be required
• The approximate treated area (acres or sq. ft.) per treatment, per customer
• Use rate i.e. oz. concentrate per finished gallon
• The amount of finished gallons per treatment, per customer
If PMP, despite following all label instructions and the requirements within this CSI ProFlex 90 Day Mosquito Protection Program, receives a complaint from a customer requiring a re-treatment, the PMP must contact their CSI Account Manager within 10 business days of the re-treatment.
If CSI determines that a re-treatment is justified, CSI will provide the quantity of ProFlex to cover the re-treatment(s) of the account, as needed, but no later than December 31, 2022. CSI will only provide replacement product to a PMP for re-application. CSI will not pay for the application or other costs associated with a re-treatment(s). CSI reserves the right to investigate each performance complaint, to verify any purchases made by a PMP, and/or modify or discontinue this Program at any time.
